After enrollment, submit the original hard copies of the following documents to the Registrar to get your student ID.

• Honorable Dismissal with informative Copy TOR
• Certificate of Good Moral Character
• NSO/PSA Birth Certificate
• Certificate of Graduation – SHS Graduate
• 2×2 photo with name tags – 2pcs.
• Student Information Form *
• Data Privacy Consent Form *
