- Admission Test Result (set appointment here)
- Senior High School Report Card (Form 138)
- Certificate of Good Moral Conduct
- Original Copy of PSA (formerly NSO) Birth Certificate
- Php 1,500.00 Enrollment fee
- Read full guidelines for admissions here
After enrollment, submit the original hard copies of the following documents to the Registrar to get your student ID.
- Admission Test Result
- Form 138 (Grade 12)
- Certificate of Good Moral Character
- NSO/PSA Birth Certificate
- Certificate of Graduation – SHS Graduate
- 2×2 photo with name tags – 2pcs.
- Student Information Form *
- Data Privacy Consent Form *
